FIELD TRIP PERMISSION

My child(ren) listed above has/have my permission to go on all 2018-19 field trips with The Fulton School. I understand that most field trips may be on a bus, but that some may be in teacher- or parent-driven cars. All field trips will be announced in advance.

Statement of Consent, Release and Indemnification: I hereby consent to the participation by my child(ren), named above, in the afore-described field trip events. I understand that these events will take place away from the premises of The Fulton School and that my child(ren) will be under the supervision of person(s) designated by The Fulton School. I further consent to the conditions stated above on participation in these events, including the means of transportation.

In consideration of my child(ren) being allowed to participate in these field trips, I hereby agree on behalf of myself and my child(ren), to release and forever discharge The Fulton School, its officers, directors, employees, agents, representatives and assigns, including but not limited to volunteer drivers (collectively referred to herein as “Releasees”), from any and all actions, claims, losses, and demands for damages, loss and injury, including but not limited to any claims for negligence, which may be asserted by me or my child(ren), or on behalf of my child(ren), arising from or relating to my child(ren)’s participation in these field trips.

I hereby agree on behalf of myself and child(ren), to indemnify and hold harmless Releasees from any and all actions, claims, losses, and demands for damages, loss, personal injury, which may arise from, result from and/or relate to my child(ren)'s participation in afore-described field trips.

If Yes, the office will contact you regarding your shuttle needs for this year.

AFTER HOURS PROGRAM (5th Grade and Younger)

Daily Lower School dismissal ends at 3:30 pm. All students 5th grade and under who have not been picked up by 3:30 pm will be sent to After Hours, which runs until 4 pm (later if prearranged). Cost is $5 until 4:00 pm; $10 if the student leaves between 4:00 and 4:30 pm; and $15 if the student leaves between 4:30 and 5:00 pm. Charges will appear on your monthly statement. If you need care past 4 pm on a regular/scheduled basis, please note that below and we will contact you to arrange.

SCHOOL STORE ACCOUNT

You can set up a pre-paid tab for your child(ren) at the 6th Grade School Store, which is open during most lunch periods and offers drinks, snacks, and light lunch items. The amount will be charged to your next school statement.

ACETAMINOPHEN & IBUPROFEN

Acetaminophen and/or ibuprofen may be administered by the school office with your signed consent based on student self-referral. Children's liquid and regular strength tablets are available. Dosage will follow the guidelines stated on the label for the student's age/weight.

UPDATES TO PARENT CONTACT INFO

We have your home address, phone numbers, and email addresses on file from the 2017-18 school year. (Note: If this is your first year at TFS, we have this information from your recent application.) If you think any of this has changed in the last 6 months and you haven't let the school office know, please list your NEW information below.

GRANDPARENT COMMUNICATIONS

We will send all listed grandparents special invitations and publications. If we don't already have the grandparent's information on file, please list it below.
